Open Door: A Pilot Day Service Program

Open Door at Southern Harbor House (SHH) is a supported service opportunity for our senior community members to be able to have access to support for activities of daily living, as well as spend time at SHH where staff, residents and guests can interact and be a part of the participant’s social network.

Open Door is best suited for an individual who would prefer a safe environment, and supervision or support with activities of daily living, such as transfers, locomotion, using the bathroom and meal preparation. Participants who elect for bathing support may do so, trained staff are available to maintain safety awareness to support individual bathing, or assist one-on-one with bathing, grooming and/or dressing.

SHH will support activities in-line with individual and group preferences, offering vital conversation and companionship with each visit. When able to do so, group activities may include music performances, poetry or current events discussions, games such as cribbage, or seated exercise.

Participants will be required to complete an application and intake assessment, have their Covid-19 vaccine and booster(s) when applicable, and provide negative PPD results (TB screening).

If you love Open Door but your budget is stretched, please don’t be deterred. We offer a sliding fee scale for income eligible participants. No one will be turned away.
